Insurance & Financial Information
Sometimes paying your bill is hard. We understand. Our office strives just as hard to help make the payment process as painless as we do your treatment. You will find below some information we hope will help. This information includes financial guidelines for patients with and without insurance and a link to a form you can complete informing us of who will be the responsible party. Completion of the Insurance Information form will allow us to pre certify and submit your insurance for you. Please keep in mind although we will assist and speak to the insurance company on your behalf, ultimate responsibility is that of our patient.
To make your record keeping easier, we also provide a free report summarizing your treatment and costs for the past year. Please feel free to ask for this report at our reception desk.
If you have questions or concerns about your billing, please talk to us. It is always better to let us know what your questions or difficulties are so that we can develop a plan to best serve your dental needs. Drs. Shaw, Ely and Dubos do not believe financial concerns should stand in your way.
For your convenience, we accept Visa, MasterCard & Discover. Also, for larger treatment plans, we offer interest free financing underwritten by Springleaf Financial.
